Hi,
In my case, I used my maiden name when we submitted our application. During the application, and since my passport was about to expire, I got it renewed with my new married name (appended husband's surname). We updated our immigration lawyer to update CIC. 
CIC will understand such circumstances when the women renew their passport names and change their surname following their husband's during the application process. But make sure to inform CIC and send them a scanned copy.
Regarding the address, you may use your current address. In case you need to move to a new home while the application is in progress, you need to inform CIC as well. Reason being is, with my case, they sent back all the pictures and proof to me so make sure any correspondence from CIC can reach you on the address you've provided them. Hope this helps. Well we received the PPR yesterday! We didn't hear anything for over 6 months and then got a nice email yesterday morning asking for my wife's passport, no update on ECAS or GCkey before or anything, just straight to PPR. The only communication we have had was on 3 Jan 2017 when we were asked to provide police certificates from Thailand and Oman were we have lived over the years as teachers. These were not given in initial application as CIC stated that for these countries: We will first review your application. If it is otherwise complete, we will send you more information on how to get a police certificate'.
Our application now says 'Your application is in progress. The next step is to conduct a background check'
Does anyone know what this actually means? Has the background check begun? 
I know all timelines are different but can't make sense of MyCIC status as it seemed to have been more straightforward before changes were made to the wording in March.

Hii...someone told me cic return the application package as incomplete if photos are not of correct size. I sent 35 mm x 45 mm photos of my husband as this is the standard visa application photo size. They ask for 50 mm x 70 mm photos with PPR for the PR card. I too am waiting! Have had no movement whatsoever on our file. Any updates we get through our MP states they have not initiated Background check or Criminal check. They quoted that although the website says 12 months processing, it could take up to 15 months. My husbands medical has expired and they were aware of this. They have not contacted him to renew the medical or to come for an interview. We are being processed in the Ukraine where he is a Permanent Resident there.
