What medical paperwork do I submit,just Section A with pic?

Ok, I have the general things in order and she completed the medical exam by requested doctor in US, but a bit confused what I am sending in now, or missing.
I have the section A form completed by the doctor, with her info and pic attached. But the Appendix C form, we can't remember if he took it or we fill it out and send with his report he gave us, the section A form and pic attached.
Any info appreciated as to what I need to send together as the medical portion.

add the greenish paper with the picture and the signature form the doctor to your application. it has a specific number, and the results will be sent to your Visa Office straight. You add the paper to the application, and once the sponsorship part is approved, your file will go the the VO - and the medicals will be matched to your file with the file number...
