Medical upfront *Inland spouse application** Open workpermit

Good day everyone , I’m doing my wife’s upfront medical for open work permit . It says on the CIC website:
When the panel physician completed your exam they would have given you an

• information printout sheet and
• IMM 1017B Upfront Medical Report form

they never gave me a form, just a document with my wife’s pic on it , the clinic logo , with IME number (this document doesnt look like an immigration document or 1017B ,just the clinic, the doctor said: “we need to complete the X-ray , then I will send the results to immigration“
now I need to complete my open work permit application , do I need to upload the document I got from the doctor or they will send me one after the X-ray ?

That's the only document you need. They will not send you another one after the X-ray. Upload the document you got.
