I am just wondering where to get the SECTION A form for the medical on cic website?
or do we call cic to order it and mail it to us. I am sponsoring my spouse and the doctor he is going
for medical kept on telling him that he needs to bring that form. I can't find it anywhere online.
It also said that the doctor is suppose to provide this form. But they keep on insisting that
they had clients who came in with Section A form.

With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:
IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)
New Panel Physician handbooks are not available yet and this might be causing confusion. Following is from the planned PP Handbook.
4.3.2 Upfront Medicals (UFMs)
UFM examinations are performed when clients report to a panel physician for their IME before a visa application has been submitted to CIC. Clients who undergo UFMs will not have a Medical Report (IMM 1017) issued by CIC, nor will their information be stored in the eMedical system.
The Medical Report (IMM 1017B UFM) is provided to panel clinics by the Health Branch. These forms are pre-populated with a Unique Medical Identifier (UMI#) and a barcode.
