VERY URGENT: Affidavit Format for DoB certificate

friends, I need urgent help!!! can someone please provide me the Affidavit Format for DoB certificate. I just have the secondary school certificates but dont have an actual DoB certificate. It is impossible to get a DoB certificate from the municipal authorities. I want to attach this affidavit to the secondary scholl certificate. Please please help.

This is good enough. You have to submit the Original DOB Affidavit with your CEC application.

Along with this Affidavit, please check if your School Certificate (which has your DOB) also has your Father's Name in it. It could be supporting doc.
Preferably also attach Certificate of Date of Birth issued by CGI.
All these could make your case strong with respect to evidencing DOB.

thanks Andy, yes I do have a school ceritificate which has the DOB and fathers name. I am not going to submit the DoB ceritificate as anyways it is based on the DOB in passport. What do you say?

Friends, can some please confirm if I need to get an affidavit from India which is notraised and signed by my parents or an affidavit made in Canada and signed by me and then notraised by a public lawyer will be good enough. Please let me know quickly.

As per my understanding, it is good both ways. In case you choose to get from India, it should be signed by your parents. If in Canada, then signed by you and notarized.

It is extremely unlikely that an affidavit is going to be accepted for proof of birth.

Hi PMM, I am having my secondary school certificates which has the fathers name and the true date of birth. I will submit them along with the affidavits.
