Super visa for mother

Hi,

So my wife's mother will be applying for a Super Visa , want to know how the medical exam work, does she have to get this done or should we wait until they tell her to get it done? I am looking here http://www.cic.gc.ca/english/information/medical/index.asp but it is unclear what to do exactly, I don't want delays in the process that is why I ask. Thanks.

also another question I have is how will the visa office know that we are applying for a Super visa and not a normal visitors visa? the forms are exactly the same?
