Police Check - Previous Countries of Residence

Hi guys,

I lived as a student in the USA less than 5 years ago and I'm slightly confused as to whether I need to request a police record check:

Aug 2017 - December 2017 - Wisconsin
January 2018 - May 2018 Kentucky

My questions are:
- As I went to my home country during that time does it count as living more than 6 months in the USA or does it count as two periods of 4 months?
- Would I need to get a criminal record check from both states or is there a central location I can get criminal record checks from?
