U cant apply through us citizen . because as per rule, ur representative should be canadian pr or citizen. and CIC only disclose ur information to them only who are PR/citizen of canada

Following form i had use for CAIPS which i received few days


1)Formal Request Letter(If you want i can paste my letter here)

2)IMM 5563 B form(2PAGES)-Access to information and personal information request

3)IMM5475 E Authority to release personal information to designated individual.


Dear CBI/Setcanada,

I sent IMM5563 and 5475 both bfor 2months ago. my representative in canada got a letter stating that u need to sent updated form for CAIPS i.e 5475 and 5476 then only we will able to reply..

i sent those forms demanded by CIC and my representative confirmed them over the phone.. they acknowledge it and said that all forms are correct and u will get reply within 6 days.

so i think that they have replaced form

I filled up IMM 5563 B &IMM5475,formal request send on 4th oct received by citizen and immigration canada on 15th oct and CAIPS issue date is 8th Nov Citizen and immigration canada never ask me for any other form & released my CAIPS link you mentioned is showing same form what i had used.

When did you apply for CAIPS?

My consultant has just sent me a mail saying that CHC doesn't issue Visa in November and December ..
and wait till Jan ..

Is it true CHC Visa officers are in a XMAS mood from NOVEMBER only...????

There may be delay in work but i dont think that CHC will have any intension not to issue any visa in these two months..
