Nope. It will happen at an inland CIC office. If you exit Canada, it can be problematic. So, just follow the landing instructions (they will assign an inland office closest to you (by postal code)).
no, because inland applicants do not receive the CoPR by mail, it's handed to you at the appointment. In which office you will land is based on your postal code. If you are in BC, you will land either in Vancouver or in Surrey http://www.cic.gc.ca/english/information/offices/help.asp#bc

it was the case for us as well. The med request came in Dec 2016, and SA, AIP and DM came only in March 2017. But as @profiler mentioned, check ECAS regularly, especially using the sponsor's information (which will give you status on both the sponsor and the principal applicant).

Hopefully! But in our case, even the GCMS notes of March 8th, 2017 didn't show SA... I don't think it's very common, but it can happen that medicals are requested without issuing SA.
However, like you, my best bet would also be that they have SA already.

Then it will mean that they have come to your file, and they want a medical to be completed, so that when they process it, the medical should be submitted.
