Which forms are required for Study Permit!!!

I'm apply for study permit from Pakistan...
I have to fill
"Application for a Study Permit Made Outside of Canada [IMM 1294]" form &
"Supplementary Information Form PDF * (38 KB)" form.

Do I need to fill "Schedule 1 – Application for a Temporary Resident Visa Made Outside Canada [IMM 5257 - Schedule" form?
or "Family Information [IMM 5645]" form?

It is stated on the website that "The application for a study permit includes the application for a Temporary Resident Visa."
What does that mean? Do I need to fill "Schedule 1 – Application for a Temporary Resident Visa Made Outside Canada [IMM 5257 - Schedule" form?

Yes you will have to fill up Family Information [IMM 5645] also.

To find out whether or not you have to fill out the Schedule form, refer to the Background Information part in the IM1294 and see whether you have YES in any of the questions from Question 3 to 7. If you have Yes in at least one of them, you will have to fill up the Schedule 1 form.

Well for temporary resident visa, you will not have to fill any other application. If you qualify for study permit, the visa will automatically be issued on your passport. Like the name says, study permit is not the visa but just a permission on a paper (it will not be stamped in the passport) to stay and study in canada while the visa will allow to legally enter canada.

Supplementary_Information_Form-EN is also mandatory. Right?

Well in Bangladesh, we don't have any Supplementary_Information_Form-EN. Maybe it is for Pakistan application or something. You should contact with the local authority or go through the link posted by abrar2007 to find it out. Sorry for not been able to help in this case

but bangladeshi Family Composition form is much more simpler than this

Yes usman you are required to fill family information form[IMM 5645] but about "Schedule 1 – Application for a Temporary Resident Visa Made Outside Canada [IMM 5257 - Schedule" form i did not submit this along with my application n i got medical...i dont think it is necessary....best ov luc............

family information form[IMM 5645] is mandatory and if some other family member is accompanying you then you have to fill "Schedule 1 – Application for a Temporary Resident Visa Made Outside Canada [IMM 5257 - Schedule" form for each member.
