I called one of the hospitals on the CIC website to book an appointment for my medical examination and I was told to come with a biodata form from the embassy. Please where can I get such a form?

Tell them you're doing this for immigration purposes as an up Front medical and they should have the form. That's what my wife did and the doctor had the form already and he uploaded it on e-medical for CIC.

We were not asked to provide and such form since it was done upfront. International passport, passport photo and the applicable fee were the requirements.
Call them again or call another panel physician.

Well my GCMS notes showed me that in November and then recently my online status showed that they were received.

Check the online status?

You should have a UCI number with your SA approval letter. Then you can check your status online by following this link and the steps

Any news on the sponsorship approval? they're working on January applicants now!

Nothing yet. Still waiting. Apparently there was a mistake from CIC. they shouldn't have sent the AOR when they did for Quebec applicants. This happened to a couple of people in the December/January thread.
Waiting is all it is.

I called another hospital and the lady asked me if I had submitted my application or whether I wanted to do an Upfront Medical. I said Upfront medical and she told me the required documents to bring. She said nothing about biodata form. Will be going on Friday.

Ok guys, what's the news? My wife sent her passport down to Accra on Monday, but it was delivered early today at around 9:00AM. So hopefully they stamp it today, tomorrow or friday. Opester, any word from your husband? Please note, we also sent a mail to the VO indicating that there's actually an envelope in the package however not a pre-paid envelope. They only have to let us know when they have finalized everything and we'll instruct DHL to pick it up.
