GUYS,,, yesterday 28 August arounf 9:30 am EST i recevied an email from CIC to submitt Schedule A for me and for my spouse

this how the message looks like
" Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide answers to Statutory Questions and provide the addresses for period between 2004/04-2005/02.
Note: The electronic version of this form is available through the following link: http://www.cic.gc.ca/english/information/applications/skilled.asp
This must be received at this office by: 2015/09/04 "
I have 2 questions ,
1- i did not find the file in the link they provided, i found it in a different link in CIC website , is it ok ?
2- do i fill out the whole peroid of time or only the time they have asked ?
