study permit extension: do I use old passport info in application form or new

Hi I'm filling out the form to extend my study permit but my previous passport expired so I got a new one, in this form do I enter my new passport details or the old one which I got my original visa and study permit in

Thanks, also should I attempt to extend the study permit until the last day of semester or until graduation so I have time to apply for PGWP with a valid study permit

Oh dear, I already applied and it was until the end of December 2019. I will have my transcripts by then so I should be able to apply for my PGWP but I think it still might be a bit rushed. Is there no way they might give me the permit for time over what I requested? I heard the officers usually consider WP time
