Is there an instruction guide for filling out Statutory Declaration of Common Law Union? When putting the date we have cohabitated for do we leave the line blank and write ongoing or write the date that the form was signed? Also, if we want one copy to send with the work permit and one copy to send with the spousal application do we need two originals notarized or will a photocopy work?

Funny, I just received a request for IMM 5409. Put the date that the notary signs the document in the 'date to' box. I'm unsure about whether you can send a copy of the statutory document or not. Can you not use the single document for both apps if you're submitting together?

It is required when applying for an open work permit. It’s not required for the sponsorship application but I have heard of a few people getting requests for one so I am including it
