Sexychill, I submitted my NBI clearance upfront then I uploaded Schedule A on the CIC website.

No, IMM 5604 is for visa office. Walang kinalaman yan sa medical

naka block yung (province/ state) pero yung text box nung District pwede akong mag-type. Ano dapat ilagay dun? kasi pinagsama ko yung Baliuag, Bulacan sa (City/ Town) eh...

I notice sa IMM: 5669 (12-2012) magkakasama yung state/ province and district but in IMM: 5669 (2017) form magkakahiwalay na sila. So, nalilito ako kung saan ilalagay... Naiba kasi yung form and yung way off filling it up.

Yung 2017 form kasi yung gamit namin kasi yun yung most recent form nila.

Your NBI clearance shows *No Criminal Record* remark; please provide the following: Certification issued by the NBI outlining all criminal charges/cases including the description of the charge, date when the charge was filed and the disposition of the charge. Own written detailed account of circumstances surrounding and leading up to the criminal charges listed on the NBI certification.

Regardless if case is dismissed or ongoing, all court documents relating to the criminal cases listed which include but are not limited to police report/blotter, court transcripts, and court judgments


Kailangan ba ng bagong nbi?
Tapos meron pa nakalagay under sa Optional Documents- letter of explanation. Na meron upload button. Diko po magets kung para saan un. Maraming Maraming Salamat po sa sasagot. Godbless.

Hindi ko lang magets yung part na "NO CRIMINAL RECORD" but then hinihingan ka nila ng explanation/ documentation regarding some cases...

Un nga din po diko magets kasi no criminal record naman nakalagay sa nbi. Ung nbi kasi kinuha pa namin nong Jan 4th. Nag request lang sila ng May 10. Saka anong letter of explanation po need? Medyo confusing po kasi.
