You have to fill and upload only those forms which are applicable to your profile. (not all.)

Yea, this is the only way to get successful submission. At the time of intake opening, you have time for only one re-checked and press submit button.

Just a quick catch up again - If one of the companies I have worked has closed what to do?
I have access to General Manager who is ready to assist in preparation of referance letter.
Should GM has to put old stamp and as well inform that the company is closed ?
any other suggestions?

I think no need to mention about company closing. Generally they don't verify about company, may be if they will verify it, most phones have only been the GM (Must write GM contact details)

Could someone provide me or indicate where I can obtain a example/template about the bank letter (proof of funds)?
I asked it to my bank manager and he has no idea how to issue it. :-(

Must be on bank letter head with contact details. Proper seal/stamp with blue ink and sign to by bank manager /official

(a) Name and father's name and Account number
(b) Current balance
(c) Date account opened
(d) Date the letter was generated
(e) Bank's seal/stamp
(f) Showing required amount Rs. ................. (In words.......) maintained with this account from date .............. to date ..............

etc etc.

And for your ease,you can also apply with 3months bank statements with duly signed and stamped by bank manager with balance certificate.

Balance Certificate(because its system generated document containing all the information which Ashu mentioned above)

Don't know what to do now. U submitted for both EE and OID and i have feedback for EE already in which i claimed the same NOC for both.....Please your advise would be highly appreciated!!!!
