While submitting full application, can I send photocopy of Forms I sent earlier (that would have 5 month old signing date) or do I need to fill it again & send new copy of forms ...

Better rewrite the forms again and also ensure if any changes are there like mailing address etc.

What date should I put on the form.... date I have filled on forms in step1 (Forms submit earlier) or current date?

Photocopies are ffine just make sure that you sign the photocopies.

pooja, Older date, Coz you applied once to CIO and what youre sending to CHC now which is the sequence of the CIO's sending. and yes, photocopies are fine if there is no change in information like shibuya said- address of others. Remember to explain any change from the previous forms.

Sign Them once again. Please read the instruction guide it is clearly mentioned there

I have read instructions guide 2-3 times in span of one year.... but replies in forum are very helpful...
