If my pgwp expired on 09-2020 but I already applied and received closed work permit, can I apply again for open work permit?

Now that you already have a closed work permit, I think you cannot apply. But please confirm with IRCC directly or wait for some expert to answer this.

I meet all those 5 criterias , there is nothing saying about having closed work permit, so I think I am available

Upfront Medical means you conducted medical before any official MR from IRCC, Yes u can do
