I have a one question. As NDVO returned my passport without stamp due to medical is going to be expire in Oct-2021 (Spousal sponsorship application), but my TRV is approved too, they returned the passport without any reason for TRV as well. There was no reason on the letter they provide. It was blank. So can i submit the passport again for TRV with upfront medical? Anyone went through this situation please advice.

Yh, I think yall can redo an upfront medicals with your previous MR sheet and redo it. Cos I feel the most important reference number is your assigned IME number which should be the same since you haven't yet been requested by IRCC. I know some pple who did a remedial even without request and later noted ircc. Got updated after a while. Once it gets updated and recognised you can then submit your passport or ask for the next step.

Can we do upfront medical for PR as well? I checked with panel physician and he also told me that they need the IME number. I have the pervious instruction letter.
And my another question is:- how can we upload the new medical or it will be automatically updated by the panel physician under the IME number.

Yup exactly without the IME number you can't so once you have one, the panel physician could do an exception for you. Explain your situation to him should be fine.
I think if you discuss with your doc he could send it to them directly as well as email you a lab result sheet as evidence. With this file or receipt you can send it via webform to your local visa office with an explanation. Or rather still email to their direct email address.
