Which of the following is TRUE -

a. Both the Principal Applicant (My Dad) and Dependant (My Mom) have to fill their individual copy of IMM5562?

b. Only the Principal Applicant fills the IMM5562 as it has column for Spouse as well?

It has column for spouse, so I cannot think of a reason why you need to fill 2x forms

Can anyone explain why they request this travel form? You supposed to put the travel history along with your address and personal history with the application.
