Hey, Have you heard back or got your file number? I am yet to receive an acknowledgement and my application was received on May 1st 2017.

Form IMM 5669 background declaration
Q6. When doing year/month for history how have you all written this?
Example below:

Option #1
2016-06--- 2016-10
2006-01---- 2016-05

Or

Option #2
2016-06-- 2016-10
2006-01-- 2016-06


Guidelines prefer option 1. But what person visited outside country mid of month (example June 15)

MY application as well got receive on May 1,2017. No confirmation letter yet.

Thanks for the reply. I tried writing "NOT APPLICABLE" but it asked for a date, I put in 0000-00-00 but it won't accept either.

Did you find any solution for this? I don't have DOB for IMM5669E Q3 so i'm unable to validate this form without this information!

I just used option 1, using where the parent was on the 1st of that month

They don't mention anything about getting documents notarized. I still sent all the documents notarized just to be on the safe side.

I had the same problem. WE don't know when was my grandfather born. know the year though. Accepted norm in canada is to use 1st Jan if date of birth is unknown. thats what i used. Don't think this will be a big deal

I have confirm with consultant with if u don't have information,u can leave blank and don't validate the form and attach letter of explanation why u leave information blank .

I have a very basic question and for some reason i cant find an explicit answer.
I need to sponsor my father and my mother. Do I send two applications with 2 principal applicants?/ sending same application but filling in principal applicant information twice?
Lastly what fee would I pay. 1050 times 2 (assuming I also pay PR fee in advance)?

Again, completely wrong, only income included in line 150 is considered for sponsorship, nothing else matters, regular EI is considered other income, yes that is true but it is not in line 150 and IRCC has been very clear and black and white, only what in line 150 is considered

financial evaluation form, information needs to fill gross income.
In self employer( noncooperation) case.does Income can be filled 0 for present half year .He May generate some income in half of this year,.does any proof required to attach in self employed case for present year.
