Out of curiosity, do I need to do medical if I am an inland applicant who's never done medical in the past 5 years?
Yes, if you have not done the medical in the past 5 years, you need to do medical.

Same situation and I had to do the medicals.

My situation is a bit different. We did the Medical about 2 years ago, we are inland, and we've applied for PR but still waiting for the AOR. My wife will travel to our homeland for 3 weeks in January. Is she going to need another Medical because of this trip?

No, re-medical is not required since your medical is just 2 years old, she will be exempted. However, just to be on the safer safe, get the PCC done again after the January travel if feasible and update via webform if you don't get AoR by then (I know about the 6 months thing with PCC n all).

Can you post your timeline?
How long did it take between AOR and Pre Arrival Letter?
37 days between AOR and Pre Arrival letter. Got my pre arrival the next day as soon I passed my MR
I believe the delay is pretty much caused by how your application was submitted. I had my application submitted by a representative. Even though the fees are really high, I would recommend going through a good agency to get your application in. With the new online portal most things are automated. So if you miss a document of things then an actuall person will have to review things and I believe this will slow down things. If you have properly submitted all required documents the the automation part will do most of the stuff for except the final decision.
Just got a ghost update. Not sure what it is.

Did you receive this ADR in your MyCIC account? I ask because the wording that you have quoted sounds like what scammers send and not the IRCC.
