Should I wait for the NDVO for the medical exam request or get it done now? I am already in Canada on study permit since May 2016.

In Generic Application form, Point 3 (How many family members are in this application), does she need to mention 1 or 2 (including me)? I'm little confused, since I am the sponsor, should she count me or herself only?
