need urgently advice reg med

hi everyone,aft the submitted the proof of funds now they send med ,but problem we got on my wife surname.on med form they uses my wife surname before marrige as state on passport front page, but later we amend surname on passport next page,we inform them so many times but they cont using old name .so now we book our med next week ,so please give us advice .or we can cont to wth old surname that already on passport front page.cause allready tooo late.
