LOST PR CARD APP FORM

Hello! iI lost my PR card and now I am applying to get a replacement. The lost PR card was just renewed last year JULY 2019 but unfortunately aincant remember the exact date. I first got my PR card when I became PR last JUNE 6, 2014.

In section H #26 of the Application for PR card form, it is asking me for the Date of Issue of the card. Should I put there the date when I first got my PR card which is 2014-06-16? Or the current date of issue of the lost one which is in 2019? But what if I cant remember what date it was issued last year since I dont have a copy of it?

You need top put in the date of issue of the card you lost. You must do your best to find the issue date of that one.

Try everything you can to get that date.
