i got my 2nd file number after 21 days of 2nd AOR...got a mail /post from them with this new file number... sent documents of 6th of Aug to the london office as i am from pakistan....got the AOR letter on the 4th of Sept with 27th sept date hand written on it.

You mean that you received 2nd AOR with the first (CIO) file number, then after that they sent another mail with the new file number. am i acorrect. please clarify.

We reviewed your application and sent you a letter on April 29, 2010. Please consider delays in mail delivery before contacting us.
Your application and supporting documents were received by the London England office. They are pending review. A decision has been made on your application. The office will contact you concerning this decision.
We transferred your application to the London England office on May 5, 2010. The London England office may contact you.'
Could anybody please help me in interpreting this and what they mean by a decision has been made on ur application and the office will contact you concerning this decision.While on the main e-cas it shows ' in process '

Decision made before the request of medicals is not a good sign, the status in-process meant they are doing background/securities check and if they go alright then they send the medical request, in your case you didnt receive the medical request but your e-case is showing decision made which means refusal. I hope its not but majority of the cases like this receive refusal, you will receive a letter in your mail box soon.
See it could be a system error as well no one can tell you until you receive any response from visa office, wait for few days and keep checking your status if you dont receive any letter send an email enquiry about your status.
Unfortunately this could be a bad sign
What I can understand from their quick reply to your case is that you must have missed some of your documents.
The first thing V.O. does is check if you have a complete application and enough documents for the V.O. to evaluate your case, if not then they reject it, if the entire document are available then they issue AOR and send it to PPS.
My advice consult a lawyer.
