PRAISE THE LORD!!! Got Aip and medical forms dated yesterday June 3... My hubby is a Sept. 30, 2013 Married Applicant. with 4 dependents, wife and 3 teen children. Thank You God!

Congrats strawberry28! did your husband receive the medicals forms for each dependant all together in one email? each dependant has his/her own med form?

does anybody here received a medical form for a non accompanying minor dependent? What can my child use as a proof identification in the absence of a passport? my child is in the philippines.. Please share if you have an experience about this. Thanks!

Thank you Margaux Me and my husband are using the same email-add... The aip is the 5 pages attachment, right? Then separate email for each dependent for the medical form with the barcode is also an attachment for each of us. All sent/received the same day. No medical for hubby because he is the LC1 correction: aip is 2 pages and email with 5 pages attachment includes the instructions for LC2 & LC1

Im sorry the aip is only 2 pages, then another email which include all the instructions for Lc1 and Lc2 is the 5 pages email..

We did the medical exams today and were scheduled for immunization interview this june 9.. Were you guys instructed to do the same? To come back on a later date for such? What if we are lacking some immunizations that would require more than one dose and should be weeks apart? Do we need to complete them for us to be cleared or is the initial dose enough?

I was given measles shots when i had my medical exam, but that was it...they said PI has a measles outbreak so the shot was a requirement...

may i ask where did you go for medical? St. Lukes Global also? how much did you pay? and about the shots, is it included with the 5,500 fee for 14 y/o & above? I am not sure about the fee yet, am I right? sorry... too many questions... :
