i just want to ask where can i get the imm 1017 e? is this form provided by panel physician in the philippines? i have one dependent in the philiipines but not accompaying me but has to go undergo medical examinations. where can she get imm 1017 e. ? thanks to all

hi MARGAUX did u received the 2nd email already for your dependents medical request? i got may aip today and the forms so are you gonna received the medical request form with uci on the next day? i have two dependents my husband who is here in canada and my stepdaughter whos not accompanying me but medical is required in philippines.

Finally got the AIP today and the medical request for me and my son. Hubby's application was received April 15, 2013.. :) wishing all of you updates on your applications as well!

I know this may come across as stupid, but I need not pass my CENOMAR since I'm the spouse of the LC1 right? Just wanted to make sure. :)
 
Cenomar is for the dependant ages 18up and single..:)
