Hi plz elaborate more for us to answer
a) I have a daughter who was PR with us, now I am confused about which section of the form I use for her, under section 5.1 or section 5.2.
b) The language used is too confusing.
c) ID - ur child must have school report card so add that along with health card - Photo ID would be her passport

Oh sure, I talked about it in the May thread. Basically in my wife's form, there was a checkbox that simply did not appear in the printed PDF. No sure why, but it was somehow cut. So they returned mine and my daughter's with a note saying they were considered complete but were returned because another family member's wasn't.
I reprinted the form, checked that the checkbox showed up and resent it this morning.
For whatever's worth, the missing checkbox was the one about allowing them to share the SIN with the CRA. Sucks that it was returned for such a small thing but I understand ¯\_(ツ)_/¯
