Just the permission release and consent form. However there is a checklist of things I need to take with me ie.
1. the notice (to appear for oath)
2. 2 pieces of identification
3. all original docs
4. PR card
5. Original Passport
6. (optional) - holy book of your choice
7. signed copy of the permission release and consent form
