Indian passport surrender

Hi ,

I'm submitting the application of family for surrender indian passport in canada and BLS have checklist for Adult application . But not sure what documents need to include for Minor/Child application.

Please can any one help on this.

Its same as adult checklist , but address proof needs to be of both parents

Thank you for reply. Also can we 3 of us send the application in one envlope (Booked one Pickup) and drop-off for 3 persons) is that work .

OR

do i need to book separately for each of them for pickup & drop-off. Please any suggestions

Checklist is the same for minor as well.
-Checklist (all pages)
-Application Form that you typed online
-Canadian passport copy and Canadian Citizenship certificate copy
-Original Indian passport and Copy of the Indian passport (current one)
-Proof of Address (Include parents proof of address)
-Photo

Although not explicitly mentioned, just include copies of both parents of the documents mentioned in red.
The copies of the supporting document you submit should be self attested.

I thought same , but in BLS contact page , all postal applivcations from Alberta, sask , manitoba go to BLS Surrey

Then go with it. Jurisdiction for Surrey is also Vancouver, so in the end it will all end up at Vancouver consulate.
