1- If iam sending the attested transcript in the closed envelope sealed with university stamp to WES by myself, do I have to fill in Academic Records Request Form by the university controller and include it in the sealed envelope or not since i am sending by myself (EGYPT)
2- For the notarized copy, assuming that the original is stamped, do i have to stamp again on the copy ? i dont understand

1. Academic Records Request Form is only required if institute is sending documents directly to WES.
2. Original is stamped by whom?

Well it says send attested photocopy, so does it mean that the original (photocopied) transcript have to be stamped or i shall stamp again on the copy ?
Original doesn't require to be stamped by notary public. Take the original document to Notary Public, normally they will make photocopy of the original document themselves, then they will notarize the photocopy only.
