If iam sending the attested transcript in the closed envelope sealed with university stamp to WES by myself, do I have to fill in Academic Records Request Form by the university controller and include it in the sealed envelope or not since i am sending by myself (EGYPT)

This request form is to help you get the required response from the University as some of them do not want to give out anything without such request. It is not necessary to include it in the package and send it back to WES.
