How to fill rprf receipt ?

1. Name (sponsor name is already written ) as he paid with his card
2.DOB of client # whose date of birth will type here.... sponsor or applicant ?
3. Client ID Identification ....what to write here?
4.Address ....hows address will come spinsor's address or applicant's address ?
5.postal code?
6. phone number ?

the information is for the person who paid. it doesn't matter whose name and info is on it.

plz help if u know

you can put sponsors details, and sponsors Id number, and address. When CIC gets it they just scan the barcode and they will match it to your application.
