How Do I send in the Initial Application to CIO

Dear All;

Would request you to clarify me on the following:

(A) Documents to be sent to CIO, while sending the Initial Application
(B) How Do I send in the Initial Application to CIO ==> Courier (Blue Dart / DHL) or through Registered Post?
(C) Do I need to attach any Covering Letter?

Please clarify, as i am planning to apply by Next Week!!!
