Help Clarify Consent Form in GCMS request.

Hi!

I need help. I am currently filling up a consent form for my GCMS request.

Section B says: If you are also seeking the information of your spouse or common law partner in your ATIP request, you must indicate their name and complete Section E.........

My question is: Do I need to fill this out with my husband/sponsor's info? I have already filled up Section C with his info being my designated individual'c contact.

Appreciate any input.
