Consent form for GCMS Note

Hello guys
I need help about consent form.
My spouse(sponsor) will apply my gcms note on my behalf and her gcms note as well.
I want to know like what name should I put in content form in Section A, B, C.
Like in section A (my name applicant name) ?
Section B ( my spouse sponsor name)?
And section C (my spouse name because she is applying for me)? Right?
If I am wrong pls current me.

No - it's super confusing. Sometimes I think they make it confusing on purpose and they do not have definitions. Honestly, I would consider myself to be educated and English is my first language but it's still confusing to me. Which means I don't have an answer.

I normally fill out Section A, C, D and F and received my notes.

I got it, but my sponsor also want her gcms note so if she need then we have to put detail in Section B AND E?
