We did our medical on 07-01-2011 & it was a very smooth experienceas below.....

1. We submitted our medical form section A, 7 Passport size (35 X 35) Photos for each, 2 Passport Copies for each

2. Paid the fee 2500 (PA) + 2500 (Spouce) + 1050 (Child below 11 Yrs)

3. Urine test

4. Blood test

5. Hight & Weight

6. X-Ray

7. Physical test of Eye, Blood Pressure

Thats it & we were free in just one hour.

The clinic is well organized & managed.

Thank to all of you for giving the right knowledge to proceed with medicals.

Hi Sm001...

Yes I have sent them to my agent and he will send those by tomorrow. Actually we are waiting for CHC reply....My agent has made inquiry regarding weather I should fill IMM 5490 form (Which is actually a Sponsor questionnaire form, For those who are already in Canada and are sponsoring their spouse.). CHC has asked me to filled that form in their letter. Anyways we have filled that form and my agent will check it today and will send it CHC by tomorrow.

Folks quick question, how do we get the Second AOR from ND office. I submitted my documents through consultant on Jan 3rd. I dont expect anything for next 3-4 months. But curious whether the second AOR is through email or through regular mail.
