You see where you have "Employer Record 1" , "Employer Record 2" etc...? Now when you submit the application, you will see the position title you entered instead of a number. My issue is that the reference letter pertaining to 1 was uploaded against job title pertaining to 2. I don't know how, I made sure that the same order in the form was followed for the upload. I really hope this is not a cause for rejection as I haven't claimed points for my wife's experience but they system still needed them.
Just send case specific inquiry to CIC regarding this and explain them. Hopefully this will be of some help.

I don't think it matters at all, even I was worried what sequence to upload because at the end of page it shows employment records1, employment records2.....
I have seen in GCMS notes, each attachement gets uploaded with a unique eDoc id in CIC system and Item type for all employment records is same "Employment records", it's not distinguished with 1,2...
They should be glancing all edocs with same item type to understand which document applies to which employment records.

So I would like to know which category I am eligible to apply and what NOC that would be.

FSW program if you obtain the minimum required English CLB level.
You have to look what NOC matches the duties for your work experience.

You need a CLB 7 in all sections of your English test (reading, speaking, listening and writing) in order to qualify to the Federal Skilled Worker Program. If you haven't taken your IETLS (or CELPIP) test you need to do so, otherwise you will be ineligible once you create your Express Entry profile.
Regarding the NOC I can't really help, you should know better than me which duties match your employment. Remember that the title is irrelevant, it is the duties the ones that are important.

You need two things. Your language test and since you studied outside of Canada you need an assessment of your educational credentials. Look at the first page of this thread for more information.
Once you have your test results and education assessment you can go ahead and create your express entry profile.

My duties and job title was same as the one mentioned in NOC #4012. I wonder if this experience which was during my grad studies would count as Canadian work experience for me in Express Entry ranking system.

Work experience during full time studies in Canada does not count as work experience.
