I have my husband's name as my last name in my passport. In my birth certificate, it is simply "Annie". There is no last name in my birth certificate. So what should I write here.

You definitely had a last name before marriage, even if your BC does not mention it (although it is weird it doesn't). Your maiden name is your last name before marriage, and this is what you need to put in this specific field in the Generic form.

Unfortunately yes. I did not have an official last name in my birth certificate or educational docs. I dont know the exact reason. But when I started working, I had put my initial as last name. Can I put the same here as well?

Technically, this is not a maiden name, so why bother put it? Unless you want to put it in order to give more support for using it in your education credential documents.
If your case is popular in your country of nationality, then I guess CIC will be understanding. However, I hope you have any document that mentions your parents names. I think this is a more critical issue for CIC than that of your maiden name.
