Passport name and Married name

Dear Friends;

I am from Bangladesh and in a very unique issue;

Problems:
1. I married to a Musilm women and as per BD Marriage ACT I need to change my name through affidavit
2. I am married since 2006 but my Passport name and Marriage certificate name is not same ( till to date)
3. That means my Previous name reflect in Passport, Certificate, Bank A/C, Job and my Affidavted name only reflect in Marriage Certificate and my wife Passport;

How I can apply to FSWP?
Can I just put my Affidavited name in IMM0008ENU_2D.pdf form in the field "Personal Detail" - 2 -"Have you ever used any other name (eg; nickname, maiden name, alias etc) ?
