I am waiting for a decision for CEC inland 2021. But I have to apply for BOWP. I am currently on PGWP. Can anyone please tell me which option to select for the type of permit? Is it - restore my status as a worker or apply for an open permit with the same employer?

If your PGWP has not expired till now, then select "apply for a new work permit" OR "Apply for a work permit for same employer" depending if you have an employment currently. To be honest, just do "Apply for a NEW work permit" because for open work permits you do not exactly need an existing job offer or LMIA.
IF you PGWP has EXPIRED then
1. STOP working IMMEDIATELY!
2. Apply using "Restore my status as a worker". Possibly consult a registered consultant OR a lawyer to avoid trouble.

No. My application does not even say that eligibility is passed. It only says until medical. That's why I had to call. To get more info since our online accounts have limited info
