Are you sure you received AOR 4 months after you submitted your application? This explains the delay. Is your application under non-express entry? Usually non-express entry applications have such timeline. If not, better to call IRCC and find out what is going on.

Thought so. Don't worry. For non-express entry programs, it usually takes time in comparison to the express entry. You should try posting this in a non-express entry thread. That will be more insightful for you.

I have a very similar timeline to you. We inland as well. AOR Aug 16, Medical pass on Oct 10.
