Correct. I referred to the quote you mentioned. I didn't change my name and they're asking the names other than the application name. I'm good. It scared me. Thanks for your reply.
I have one question about FBI - Identity History Summary Request Confirmation
When I submit my request to FBI, I forgot to include the mail option, so that I only have pdf electronic version, and then I print it out.
Is it OK to use the print out version? I remembered the checklist mentioned it should be original version.

I am preparing my application. Which way it is better to send our application, by Canada Post or by courier, such as UPS or FedEX? I read in other threads that sometimes they don't receive the application package and people need to send them again after 5-6 months.
hi friend I am not sure still because in the questionnaire its saying the application 5(2) is for people did nt stayed in Canada for atleast 3 years right? we stayed in Canada for the last 5 years.
