The letter which comes with Medical Report section A .. A small one page letter/doc on which its written that this letter needs to be attached when resending passport. Failure to do so will result in processing delays. Unfortunately me and inam didnt received that letter with Medical Report Section A . :S

What that letter states............actually i don't remember that i specifically attached some letter with passport, how ever on asking by Gerry's staff i attached some photocopies of forms with passport send to me along with medical Section A. May be the said form is one of them..........

The letter states that you must send it back to CHC with your passport otherwise your case might get delayed.
