i got request to submit Schedule 4 and 4a today, Next day after the AOR.

i am PNP applicant. Do i suppose to fill both forms?

You should fill schedule 4. If schedule 4S doesn't apply to your case, you can attach a letter of explanation instead.

So, it is ... "getting the medical status changed to" ... Not needed at this time / Started / Passed / Failed (whatever your current medical status is).
