Congrats on the PPR ....took around 5 months and a week ?
And I dont think they would have to transfer full file but maybe just internal communication during the security and criminality checks.... no one knows what the hell is going on tbh lol .... some people applying from late Dec got PPR in Feb.... no one knows how CIC handles things...
I need to know in the PR Application, in the passport document, do we have to upload complete passport with all the pages including the blank pages? Also, do we have to upload the previous cancelled passports as well?

It depends on what CIC is asking for ....check what they are requiring ....for most they ask for just the bio page but for some cases they ask for full pages or all used pages (with visas and stamps).

and GCMS notes are the details of your immigration file and what notes officers made on it and such ..... can be helpful to make sure there are no negative issues delaying your process or to estimate when you would get PPR...

That is up to you to be honest ....if you think there might be something negative stuck and you might need to clear up then ordering notes would be helpful .... and there are two opposing theories , one saying ordering notes leads to some movement in you file and another saying it might delay a bit... and a third saying it has no effect which is the most likely one.
Notes usually reach in 35 days but the third party sites tell you 45 just to give themselves time .... 30 days if you get it directly from CIC through a friend in Canada...
One should not order notes before 90-100 days ... you would not find anything useful before that.... your current timeline is a good one for ordering notes but that is your decision...

I'm not sure what role LVO's play in the process. Maybe as you say, some applications are transferred to other VO's depending on workload distribution. I think mine happened to be processed in Ottawa. I applied on the 29th of Sept, 2015. and I received my PPR on the 2nd of March, 2016. My passports were received by the Canadian Embassy on the 6th of March. Havent received my passport yet. No updates since then. Just waiting...
Good news. It depends on the countries you visited. For me 3 months 5 days. I believe due to new passport for my family and i traveled Europe four countries and Middle east two countries. I am in Qatar for almost 14+
