The update you see in your profile is not the accurate one. They would've already extended your medical validity. Try raising a Webform to see if they respond (They are busy with the Afghan crisis now and may not respond). But if they extended it, it would be accurately reflected in your latest GCMS notes.

A medical assessment is the review of an applicant’s medical file by a Citizenship and Immigration Canada (CIC) medical officer or delegated staff. The validity period is 12 months from the date of the last medical assessment.
Medical assessment could be date the medical practitioner assessed you. My gcms notes has expiring as 28 Dec, the date the medical Practitioner uploaded the medicals. My profile shows medical passed 28 Jan 2021. Based on gcms notes I'm expecting medicals to expire 28 Dec. Hopefully they will extend them.

Medical practitioner assessing you is called as a "Panel Physician" in IRCC terms. The person validating the results from a Panel Physician is the Medical officer (Either from CIC or Health Canada). In 99% of the cases, the validity of the medical starts on the date it shows as "Passed" in your EE profile ...
Only in very rare cases, the validity start date would vary. Else in most of the cases, it would be valid for a year from the date its results are processed by a Health Canada Officer.
No, I think in most cases, it expires either the day it was done/the day it was sent to IRCC. It doesn't expire when it was passed. My gcms notes say my medicals are valid until Dec 9, 2021, and again, this is a few days after I did my exam. I assume this was the day the report was sent to IRCC. This is the same case with most people with expired medicals.
If you really wanna be sure when your medicals will expire, just check your gcms notes.
