How long do they approve extensions for these days? Is it always one year at a time? My parents arrived on August 2019. Visa officer allowed them two year stay.

July/August 2021 depending on when they arrived. Extension times vary. 1 year is common for a first extension. You can certainly ask for a certain time period if you want although you may not get it. Supervisa remains a visitor program so parents should be planning on returning home at a certain point.

Yes you need to show proof of income, proof of insurance, etc. Nothing is guaranteed but the first approval is fairly common as long as everything checks out. Another medical will be required.
