Yes you have to list all names including name at birth. Refer Citizenship guide for more info.

my wife is eligible for citizenship she complete her time in november 2019, but we didn't apply fro citizenship and went abroad urgently, we were thinking to comeback after 3 months then we will file the application now we are stuck in pakistan can't com back due to Covid-19, can she apply for citizenship from pakistan?. thank you
